Care Coordinator

About the role

As Care Coordinator, you will be responsible for the day-to-day care of a group of elders: coordinating home visits, supporting our field care professionals (CareMates), keeping families informed, and making sure every elder in our care is well looked after.

The role is based in the field. You will visit elders at home every week, meet CareMates on the ground, and accompany parents to appointments where needed, alongside coordinating with the clinics and labs families already use. It is a coordination and care role and does not involve clinical procedures.

Key responsibilities

• Home visits: visit elders at home every week to check on their wellness and the quality of their care.
• Visit coordination: schedule home visits between CareMates and families, and make sure every planned visit happens.
• CareMate support: guide, support, and coach the CareMates who carry out visits.
• Visit oversight: review visit notes and wellness check-ins on the Zeraphos platform, and act early on any concern.
• Family communication: serve as the main point of contact for families abroad, with regular updates by phone, WhatsApp, and visit reports.
• Appointments and follow-ups: arrange accompaniment, lab follow-ups, and doctor appointments with the clinics and labs families already use, and report back on the outcome.
• Escalation: recognise when a concern needs escalating, and escalate it promptly.
• Records: keep accurate notes of every visit and family conversation.

Requirements

• A background in care: nursing, home care, eldercare, or healthcare.
• At least 1 year of work experience in nursing or care, not counting internships.
• Willing and able to make regular home visits in Ernakulam and nearby areas.
• Dependable and hands-on: you follow through on every visit and every commitment to a family.
• Sound judgement and the ability to stay calm and act when an elder's situation changes.
• Clear communication: in person, by phone, and in writing.
• Fluent in Malayalam and English.

Preferred

• A nursing qualification (GNM, BSc Nursing, or ANM).
• Geriatric, eldercare, or home care experience.
• Experience working directly with patients' families.
• Experience coordinating with hospitals, clinics, or diagnostic labs.
