Care Manager

The role

As a Care Manager, you'll own the day-to-day care experience for a group of elders: coordinating home visits, supporting our field care professionals (CareMates), speaking with families, and making sure every elder in our care feels seen, heard, and looked after.

This is an on-site role based in Thiruvalla. Some days you will be out visiting family homes and meeting CareMates in the surrounding area; others you will be coordinating from base, working with the clinics and labs our families already use.

Who we're looking for

Zeraphos is changing how families care for their parents from a distance, and we are setting the standard for what that care looks like in India. This role suits someone with high energy and a startup mindset who wants to build it with us over the long term.

What you'll own

• Care quality for a portfolio of elders: you're accountable for how well each elder is looked after.
• Day-to-day care delivery: coordinating home visits between CareMates and families, and arranging appointments with the clinics and labs families already use.
• Family relationships: you're the trusted point of contact for NRI families, keeping them informed through calls, WhatsApp, and visit reports.
• CareMate coordination: scheduling, supporting, and coaching the CareMates who make visits happen.
• Visit oversight: reviewing wellbeing check-ins and visit notes on the Zeraphos platform, and acting early on anything that looks unusual.
• Appointments and follow-ups: making sure accompaniment, lab follow-ups, and doctor appointments actually happen, and that families hear what came of them.
• Continuous improvement: helping us raise the standard of care, week over week.

Requirements

• A background in care: healthcare, home care, eldercare, or nursing.
• Strong judgment and the ability to recognize when to escalate a concern.
• Excellent verbal and written communication skills.
• Fluent in English and Malayalam.
• Comfortable using WhatsApp, email, and standard office software, and able to pick up new tools quickly.
• Experience working directly with families is preferred.

Nice to have

• Prior experience coordinating with hospitals, clinics, or diagnostic labs.
